Add a Dependent

Add your new family member to your UTSW insurance, set up a flex spending account (UT FLEX), review life insurance options, or change beneficiaries at My UT Benefits.

You have 31 days from the birthdate or adoption date to complete changes to your benefits. If you do not make your changes within this 31-day period, changes cannot be made until the next Annual Enrollment in July, to be effective the following September 1.

Possible Next Steps

  • Gather Required Documents

    For biological children, ONE of the following documents:

    • Birth Certificate of child proving relationship to subscriber
    • Certification of Vital Records proving relationship to subscriber
    • Verification of Birth Facts form proving relationship to subscriber
    • Medical Support Order requiring subscriber to provide medical coverage
    • Paternity test accompanied by Court Order, Medical Support Order, or reissued Birth Certificate

    For adopted children, ONE of the following documents:

    • Court Order of Adoption
    • A Pre-Adoption Placement Order issued by a Licensed Child Placement Agency
    • Court Order naming subscriber as Managing Conservator of Child
    • Birth Certificate of child with adoptive parent(s)
    • Medical Support Order requiring subscriber to provide medical coverage

    For stepchild, BOTH of the following documents:

      • Birth Certificate of child; and
      • Marriage Certificate of subscriber and spouse (biological parent)

    For foster child:

    • Court Order establishing a parent-child relationship between subscriber and foster child
  • Add New Family Member
    • Go to My UT Benefits.
    • Click “Dependents” in the left menu.
    • Click “Add Dependent” to provide and save dependent information.
    • Click “Life Event” in the left menu.
    • Select the reason for the change.
    • Enter the date of the event. (Your coverage change will be effective the first of the month after the event date.)
    • Follow the prompts to select coverage changes.
    • Save your changes.
    • Review your Confirmation Statement once complete.
    • Click “Document Center” to upload the required documentation.
  • Upload Documents
    • Go to My UT Benefits.
    • Click “Document Center” in the left menu.
    • Click “+ Add Document.”
    • Click “Choose File” to select your document.
    • Enter a document name.
    • Select the Category from the drop-down menu.
    • Save your document.
    • A document should be associated with each coverage or dependent change.
    • Once you have uploaded your document(s), click “Associate a Document” to attach the existing document upload to the pending requests.
  • Set Up A UT FLEX Account Profile

    UT FLEX is a flexible spending account (FSA) that allows use of pre-tax dollars from your paycheck for qualifying, out-of-pocket health care expenses with a Health Care Reimbursement Account (HCRA) or qualifying dependent day care expenses with a Dependent Care Reimbursement Account (DCRA).

    • Go to the UT Flex Employee Portal
    • Click “Sign In” in the upper right corner.
    • Click “Register.”
    • Your BID is the eight characters after the UTS0 on your Blue Cross Medical ID card.
    • The Employer ID is BBB132002030.
