Lost or Stolen Media or Devices

Report lost or stolen devices to safeguard protected health information, credit card numbers, intellectual property, and more.

Lost or stolen computers and other devices must be reported immediately to safeguard sensitive information including:

  • Protected health information (PHI)
  • Personally identifiable information (PII)
  • Credit card numbers
  • Intellectual property

Complete required forms as thoroughly as possible for the lost/stolen media or device. All information provided will assist in investigating the device and identifying the nature of the data exposed. 

Instructions

  1. Contact the local police department if the device was stolen from an off-campus location.

  2. Report the lost or stolen device to University Police regardless of whether the loss was on or off campus.  Complete and submit the Report a Crime form or call 214-648-8311.

  3. Complete the Lost/Stolen Media or Device Form in UTSW ServiceNow.

  4. All state-issued items must be reported as lost/missing/stolen per Comptroller rules. The Departmental Asset Administrator should complete a Missing or Stolen Property Report for State Departments, Institutions and Agencies Form (VPN Required). Forward the completed form to the Asset Management Administration Office.

  • Hand-held computers
  • PDAs (personal digital assistants)
  • Cellphones/smartphones
  • USB drives
  • Memory cards
  • Servers
  • DVDs
  • CD-ROMs
  • Biomedical devices
  • Printers, copiers, fax machines
