Length of Stay (LOS) Index

  • FY25 index: 0.89 (Target ≤ 0.90), achieving the annual goal and improving from 0.99 in FY24.
  • Monthly highlights: The lowest month was April at 0.66. October, November, and August each reached 0.77, and February was 0.72.
  • Trend: Met the target goal for the first time. Continued year-over-year improvement, successfully reaching a LOSI of 0.89 in FY25, a significant decrease from 1.03 in FY22.

Mortality Index

  • FY25 index: 0.68 (Target ≤ 0.90), remaining below the target.
  • Achievements: Performance stayed consistently strong throughout the year.
  • Trend: Results remain stable over time, with FY24 at 0.45, FY23 at 0.51, and FY22 at 0.68.

30 Day Unplanned Readmission Rates

  • FY25 rate: 6.97% (target ≤ 8%), remaining below the goal.
  • Achievements: FY25 continued strong performance, sustaining readmission rates well under the benchmark.
  • Trend: Steady improvement over prior years, with FY24 at 6.78% and FY23 at 8%.

Discharge Efficiency and Planning

  • Launch of the Prepare for Discharge Tomorrow Order

New Order Launch from the department of Neurosurgery – “Prepare for Discharge Tomorrow” Order

  • After a year of development, the Neurosurgery Department with the help of nursing informatics and the EPIC team have successfully built and launched the “Prepare for Discharge Tomorrow” order in Epic.
  • This order has been piloted within the neurosurgery units, allowing physicians, residents, and APPs to place it one day prior to the patient’s anticipated discharge.
  • The tool enables nursing staff to prepare for discharge the night before, serving as a clear notification and communication mechanism so that all team members are aware that the patient is ready to go home by noon the following day.
