The Orthopaedic Surgery Department is dedicated to supporting you throughout the onboarding process, helping you settle in and smoothly transition into your new role as a faculty member.
Below is a comprehensive checklist detailing essential tasks and milestones to support your successful integration into UTSW, specifically the Department of Orthopaedic Surgery. From pre-arrival preparations to ongoing professional development, we are here to provide the resources and support you need to be successful in your new role.
Onboarding Essentials Q&A
What materials do I need to complete the Provider Request Form and begin the credentialing process?
Licenses/Certifications Required:
- Active TX Medical License - If you do not have an active "TX Medical License", go to: https://www.tmb.state.tx.us/ to apply. Create a "My TMB" account (required). New applicants will have to complete the online "Jurisprudence Exam" https://www.tmb.state.tx.us/page/licensing-jp-exam.
- Active DEA w/Texas address - Please Apply for DEA and provide proof of application receipt. Please do not request an address change to Texas until on-boarded.
- Board Certification documentation - Required certification.
- Life Support certifications (BLS, ACLS, PALS, ATLS, NRP) - Please provide copies of all active life support certifications. Must be completed from an American Heart Association approved course.
- ABOS certification - Provide letter or certificate of completion.
Supporting Documentation Required:
- Professional photo headshot - Provide a high quality color photo.
- Professional liability coverage - Also referred to as Malpractice Certificate or Certificate of Insurance. Provide the past five years of current liability coverage.
- DD-214 Form - N/A if no military background.
- Case/Procedure logs - Provide case/procedure logs for the past 24 months in PDF format.
- MAKO privileges - Provide documentation of training or experience for each modality requested is required.
- Hosptial affiliations - Include hospitals, surgery centers, assistantships, corporations, military, and government agencies.
Additional Documentation:
- Home address
- Driver's license/passport
- Social Security card
- Medical school certificate
- Place of birth
- Vaccination records, flu shot documentation, Tdap, etc.
- Signed Delineation of Privileges from current employer
- Address history - Provide the past seven years.
- Work history - Please provide employment gaps if any, with dates and why.
- Peer professional references - Provide three references including email, phone number and degree (no relatives, current partners, or associates in practice).
- Program director(s) - Provide Residency and Fellowship director's name and email.
What do I need from my department?
- Person Number

- Designated faciltiies assigned/locations, mail office, secondary office, etc.
What do I need if I will be participating in research?
- Complete CITI Training at https://about.citiprogram.org/ under UTSW affiliation
- Human Subject Protection (HSP)
- Health Insurance Portability and Accountability Act in Research (HIPAA)
- Good Clinical Practice (GCP)

What if I'm relocating to Texas?
Work with your Department Administrator for relocation process.
How do I get set up with a UTSW email account?
Efrain Solis, Department Administrator will set up your email (typically only 60 days before your hire date).
How do I gain access to the UTSW internal network/intranet and VPN?
When your email address is ready, you will be able to set up VPN and two-factor authentification. To access MyUTSW and internal applications, please use the GlobalProtect VPN client. Clinical users should use Citrix Workspace. More information can be found on the ForEmployees: Information Security website.
How do register for the State of Texas Prescription Monitoring Program?
