Gift by Donor’s Agent or Guardian before Donor’s Death
Donate your loved one for medical education and research.
A Donor’s Agent or legal guardian may arrange for body donation to the Willed Body Program prior to the donor’s death. This allows individuals to make a meaningful contribution to medical education and scientific research.
To proceed, please request the Agreement for Donation to the Willed Body Program found in the Willed Body Becoming a Pre-Registered donor tab. Please contact our office if you have any questions.
You may donate your loved one to the Willed Body Program prior to death if you are the Donor’s Agent or Guardian by submitting a copy of the relevant court documents.
Important: Please Read Before Continuing
If this is a time-sensitive situation, please call us directly at (214) 648-2221 option 1 so we can initiate a pending application, which allows us to collect and hold the potential donor’s information until the time of death. Pending applications are intended for individuals who may be on hospice care or experiencing imminent death, please contact our office immediately for guidance and next steps.
Agreement for Donation to the Willed Body Program
Required Sections for Completion
For donations arranged by a Donor’s Agent or Guardian, please complete Sections 1, 2, 3, and 5 only of the agreement packet.
Required Documentation
- For Donor’s Agents:
A copy of the Medical Power of Attorney or other legally binding documentation signed by the donor must be submitted.
- For Legal Guardians:
A copy of the court order granting guardianship must be attached.
