If the study involves MRI, be sure to include the Additional Language for Studies Involving MRI Procedures.

Some documents include footers that are designed to allow for electronic "stamping" of the IRB Tracking Number (STU#) and Approval Dates.

Protocol Template (Cancer Related)
  • Research is cancer-related.
  • Required by PRMC at Simmons Cancer Center.
Protocol Template (Non-Cancer Related)
  • Research is not cancer-related AND if you do not have a separate protocol
  • Not required for chart review studies
