Description:

As discussed by telephone on October 16, 2013 between 5:30pm and 6:00pm, I am formally requesting all video of the first floor of Zale Lipshy on the evening of October 13, 2013, between the hours of 5:00pm and8:00pm.  Beginning at the entrance doors leading from the valet parking along the first floor corridor up to 200 ft past the bank of elevators.  I am particularly interested in the videos involving the beating I received by UT Southwestern Medical Police that took place in the seating area closest to the bank of elevators near the valet area.  I fully expect these videos to be unedited and time stamped.  I expect these videos to be delivered to me on later than 2 weeks from this date.  I further request all video or still pictures of the exterior part of the building nearest the valet parking at Zale Lipshy where I received more beating and harsh treatment by UT Southwestern Medical Police on October 13, 2013, between the hours of 5:00pm and 8:00pm.  My expectations are that those tapes/still photos will be unedited and time stamped.  Any alterations, omission or missing minutes will be construed by my legal advisor as non compliant and the appropriate necessary actions will take place.  Further more, I am requesting video/still photos of Zale Lipshy 4th floor Psychiatric Unit Otober 11, 2013 between the hours of 6:00 pm and 8:00 pm.  Dr. Acosta and Mr. White spoke to my wife on October 14 and told her that I was banned from visting because of harassment of patients and attacking a staff member on October 11, 2013 and an alleged physical altercation on October 13 resulting in my arrest (days later for a made up charge initiated by Danny White) after Mr. White's weekend off.  However, I was allowed to visit several times over the weekend without incident.  "...and generaly regarded by staff and patients as generous, kind and caring man.  I believe that Dr. Acosta having never spoken one word to me or providing one non-employee witness making the medical decision to deny my wife the one bit of security and love that she needed most at this difficult time as unconscrionable and medically reckless at best and grounds for dismissal at worst.
