Already a patient at UT Southwestern?

Welcome to the UT Southwestern COVID-19 Vaccine Registration portal. We are working hard to ensure that all Texans who wish to be vaccinated have the opportunity within the limits of the supply of vaccine provided to UT Southwestern.

We are following state and federal guidelines and currently vaccinating Texans who meet Phase 1A or 1B criteria, or have a medical condition outlined by the U.S. Centers for Disease Control and Prevention as carrying increased risk for severe illness from COVID-19.

Additionally, education and child care personnel who meet specific criteria are now eligible to be vaccinated in Texas. The Texas Department of State Health Services’ COVID-19 Vaccine Information webpage lists the requirements, based on the federal directive, which include:

  • Those who work in preprimary, primary, and secondary schools
  • Those who work in Head Start and Early Head Start programs (including teachers, staff, and bus drivers)
  • Those who work as or for licensed child care providers, including center-based and family care providers

If you meet the criteria and are not yet registered as a UT Southwestern patient in MyChart – our secure, online health management tool – we ask that you please complete the short questionnaire below.

We need to collect some basic information about you so we know how to contact you regarding your vaccine and how to advise you based on your health background. We have taken all necessary safeguards to protect the privacy of your personal information.

 Registration Paused

Thank you for your interest in receiving a COVID-19 vaccine at UT Southwestern. Registration is now paused so that we may efficiently vaccinate more eligible individuals from our list and provide those who sign up with confidence that they will be vaccinated within a three-week window. Our registration will open and close weekly to reflect the amount of vaccine we receive from state and federal authorities. Please check back every Monday morning and visit our website for updates.

What happens next?

COVID-19 vaccinations are by appointment only. After completing the registration survey, you will be sent a MyChart activation code. You should receive an mychart@utsouthwestern.edu from MyChart within three to four business days. If you do not see that message, please look in your spam folder. Once you register and log in to MyChart, you can begin searching for available vaccination appointments. Keep in mind that scheduling is contingent on vaccine supply, which is limited. While slots fill quickly, we will continue to update our schedule as we receive more vaccine.

How soon can I get vaccinated?

Our registration portal will stay open until 10,000 people sign up. Once we reach that point, we will briefly pause registration so that we may efficiently vaccinate more registered eligible individuals on the list and provide all those who sign up with confidence that they will be vaccinated within a three-week window. Our registration will open and close weekly to reflect the amount of vaccine we receive from state and federal authorities. Please check back every Monday morning and visit our patient care website for updates.
