Patient Notification

UT Southwestern is notifying patients treated by UT Southwestern physicians of improper disclosure of protected health information. On February 6, 2024, it was brought to the attention of the UT Southwestern Privacy Office that protected health information was shared with an unapproved third-party vendor called Code Kick AB, an online project management tool. Information including patient name, date of birth, medical record number, phone number, address or ZIP code, type of planned procedure, date of service of planned procedure, provider’s name, and insurance benefits information was unintentionally disclosed to the third-party vendor in an effort to track the completion of necessary insurance verification and gather any necessary prior authorizations. As soon as UT Southwestern became aware of the issue, we removed the information from the online tool and conducted an investigation. Based on our investigation, we do not have reason to believe that this information has been further compromised, has been used inappropriately by the third party, or shared outside the third-party software tool.

Receiving a notification does not mean that you are a victim of identity theft. The Federal Trade Commission (FTC) has developed several consumer alerts explaining how to minimize the potential for identity theft. We encourage you to visit the FTC website and review its recommendations. Additionally, please carefully review information that you receive from your child’s insurance company, including any Explanation of Benefits or “EOB” forms. Check for any unusual items such as a provider name that you or your child has not seen before or a medical service that you do not remember receiving. If you have any questions about your or your child’s medical records or services from another institution, please call those providers directly.

We want to reassure you that the protection of all UT Southwestern data is a top institutional priority and that this incident is being handled in accordance with UTSW policies and related regulations. We deeply regret the occurrence of this incident and any worry, distress, or difficulty that it may cause.

Beginning Oct. 24, 2022, based on the low level of COVID-19 transmission in our community, masks are no longer required within most UT Southwestern clinical environments if you are symptom-free, but they remain a welcome precaution. Masks should continue to be worn by patients and guests visiting our Cancer Center clinics, Transplant clinics, Bone Marrow Transplant Unit, and Emergency Department.

UT Southwestern está notificando a los pacientes tratados por médicos de UT Southwestern sobre la divulgación indebida de información de salud protegida. El 6 de febrero del 2024, se informó a la Oficina de Privacidad de UT Southwestern que la información de salud protegida se compartió con un proveedor de terceros no aprobado llamado Code Kick AB, una herramienta de gestión de proyectos en línea.
