Welcome to Electronic Annual Enrollment 2008

Important UTFLEX Reminder: If you are a new participant and would like to enroll in UT Flex, or you are a current participant and would like to continue to participate, you must re-enroll to have UT Flex for the new fiscal year 2008-2009.  When enrolling be sure to indicate the ANNUALIZED AMOUNT for your election. If you use the debit card, you must re-elect it each time you enroll, even if you already have the debit card.
  • When adding or changing dependent information, please make sure to add or change the dependent information first on the Personal Information Screen before clicking the Benefits Enrollment Button to start your enrollment.
  • Please complete the appropriate Evidence of Insurability (EOI) form as shown on the Benefits Summary Screen for the following changes:
    • Add a dependent to UT Select medical coverage (Complete Fort Dearborn Life EOI)
    • Add or increase life insurance (Complete Fort Dearborn Life EOI) or (Standard EOI if Faculty)
    • Add short-term or long-term disability (Complete Hartford EOI) or (Standard EOI if Faculty)
    • Add Long Term Care (Complete CNA Application through CNA Website)

Upon completion of the EOI form, sign and date the form and mail the completed form to the address indicated on the form.  It is recommended that you should retain a copy of the completed form for your reference.

When adding or changing dependent information, please make sure to add or change the dependent info first on the Personal Information Screen before clicking the Benefits Enrollment Button to start your enrollment.

• Evidence of Insurability (EOI) is required if adding a new dependent to the UT Select medical plan. However, if you provide written proof that your dependent has current group insurance coverage, the EOI requirement may be waived. Written proof should be received in the Benefits Office by July 31, 2008. Please contact the Benefits Office if you have questions.

• Please complete the appropriate Evidence of Insurability (EOI) form as shown on the Benefits Summary Screen for the following changes:
      • Add a dependent to UT Select medical coverage (Complete Fort Dearborn Life EOI)
      • Add or increase life insurance (Complete Fort Dearborn Life EOI) or (Standard EOI if         Faculty)
      • Add short-term or long-term disability (Complete Hartford EOI) or (Standard Long-         Term EOI if Faculty)

• To waive medical coverage, please complete the Waiver of Health Insurance Form along with the appropriate documentation as specified on the Waiver of Health Form and submit all documents to the Benefits Office. You may access the Waiver of Health form on the Benefits Summary Screen of the electronic enrollment form.
