How to Determine a Cost

Your rate will be quoted to you on a “per employee per month” basis.  The rate depends on the model you select and the number of employees you intend to provide this service to.  The costs for services can vary considerably depending on your plan design, but in 1995 the average annual cost of services per eligible employee nationwide was estimated to be about $22.00.*

Information that will be helpful to us in providing you with a service package includes:

• The drug-free workplace or equivalent policy statement
• Information about the health benefits structure of your organization
• General information about the workforce including number of employees, supervisors, work sites, job categories and work type
• Services to be included
• Budget range or prior allowance for this benefit type
• Reports that will be required
