Instructions for O'Brien Center Request for Pilot and Feasibility Proposals:

All proposals must be submitted by email to Dedrian Copeland in Word format.  In addition, a hard copy should be mailed to Ms. Copeland via campus mail (mail code 8856).  Download only the following form pages:

• Form Page 1: Face Page
  • The sections for animal and human subjects can be "pending"
  • No signatures are required
    
• Form Page 2: Summary, Relevance, Project/Performance Sites, Senior/Key Personnel, Other Significant Contributors, and Human Embryonic Stem Cells
  
• Form Page 4: Detailed Budget for Initial Budget Period
  • Budget justification is required
    
• Biographical Sketch Format Page
  • Required for all key personnel
    
• Other Support Format Page: Example
  • Required for all key personnel
  • Include Active and Pending and relevant Completed
    
• Resources Format Page

THE FOLLOWING DO NOT HAVE FORM PAGES;
HOWEVER, YOU MUST STILL INCLUDE THEM IN YOUR PROPOSAL.

• Research Proposal
  • The Research Proposal must follow the PHS398 outline, but must be no more than 5 pages long (references are not included, but figures are).
• Letters of Support
  • These are required from all consultants and collaborators
