Appointments:

All services are provided by appointment and can be made by calling (214) 645-8500.

Prescription Refills:

To have your prescriptions refilled, it is most efficient to have your local pharmacist fax a refill request to the clinic. The fax number is (214) 645-3775. Whenever you call the clinic for a refill, you will be asked to leave a message and to provide the name, dose, and the frequency of the medication. Also, please provide the pharmacy's name, telephone number and fax number.

Telephone messages:

All messages are given to the appropriate clinic personnel as soon as possible, and every attempt is made to return telephone calls within two hours. So we can reach you quickly, please confirm your day time and evening phone numbers with the clinic staff routinely.

Appointment Cancellations:

If it is necessary to cancel an appointment, please let the clinic staff know at least 24 hours in advance. This will allow us to accommodate other patients who may need the appointment time.

Financial and Insurance Questions:

While several of our providers participate with many managed care plans, some providers who practice at this location do not participate in any managed care plans. In these cases, you will be asked to pay the full fee at the time of your appointment and you will be given a receipt so that you can file the paperwork with your insurance company to receive your out-of-network benefits. For participating providers, please provide us a copy of your insurance card for benefits verification.
