Cancer Information and Referral Services

Our Patient Navigators are registered nurses who have a depth of experience in caring for cancer patients and families. They are committed to assisting all patients through the complex health care system in order to assure coordination of their care.

Regardless of whether your questions are about cancer screening, diagnosis, treatment options or follow-up care, they work to ensure timely delivery of services with respect, compassion and confidentiality.

Our patient navigators:

• Provide comprehensive information about disease, treatment options, medical tests and health information to empower the patient to make informed health care decisions.

• Facilitate referrals to local and national specialty physicians and cancer programs.

• Assist the patient and family in accessing community, university-based and national clinical trials.

• Ensure that all patients have access to a survivorship care program.

• Enhance access to information and resources that empower the patient and family to obtain appropriate follow-up care after cancer treatment is concluded.

• Provide community health education programs on cancer prevention and cancer screening.
