Frequently Asked Questions: What do I do if I lost my badge? Use form RSF-026 (Lost Radiation Badge Report) under "Quick Links" to the right. What do I do if I'm pregnant? Our office understands there are questions and concerns for an employee who becomes pregnant while working in a radiation area. The Environmental Health and Safety office has provided the "Pregnant Employee's Guide to Radiation" (see Quick Links to the right) to help answer some of these questions and concerns. If you have any further questions, please do not hesitate to contact our office. If you are uncomfortable letting your management know you are pregnant, you can contact our office and we are usually able to accommodate this situation while having your fetus monitored for radiation exposure. This is accomplished by either sending a fetal badge directly to you or having you come to pick up a fetal badge at our office. To receive a fetal badge, you will need to fill out the RSF-025 (Pregnancy Declaration) form (see Quick Links to the right) and submit it to our office. I am a new badge monitor, what do I do? A badge monitor is the person selected from a department to assist the Environmental Health and Safety Office in the distribution and collection of radiation badges. You are the liaison for the staff that wears radiation badges and our office. This is an important position and your responsibilities are as follows: 1. Distribute and collect radiation badges For general instructions, refer to "Radiation Badge Monitor" under Quick Links to the right. For all required RSF forms refer to Quick Links to the right. What do I do with dose reports? When the dose report is sent to the badge monitor, they will post it in a designated area that the staff has the ability to access. The current report will stay posted until a new dose report is issued. Old dose reports are maintained for three years by the badge monitor; this is important because during inspections the inspector can ask to see the old dose reports for the department. How do I read the dose report? See the "Landauer Dose Report" example under Quick Links to the right. Should I be wearing a radiation badge? See "Guideline to Dosimetry Assignment" under Quick Links to the right. Do I wear my radiation badge over or under my lead apron? See "Proper Wear Location for Dosimetry Badges" under Quick Links to the right. Do I have to see you when I terminate employment? Only if you've had a badge or ring within the past 5 years. Requesting my social security number is against the law. Why is it required on the radiation badge request form? Disclosure of your social security number is required to track occupational exposure to ionizing radiation, as mandated by Federal and State law to guard against dual participation in the badge service. Using a social security number prevents confusion of applicants with similar or same names.
