General Admission:
Complimentary (no CME provided)

Three-day Conference:
Before September 9, 2011 – $135
After September 9, 2011 – $150
October 6, 2011 – $185

Enrollment is confirmed upon receipt and payment of registration fee. We cannot guarantee after October 6, 2011. After September 9th, the early bird registration price will end. Register before the Early Bird deadline to take advantage of the current low price.

Refund Policy

A $50 handling fee will be deducted from cancellation refunds. Refund requests must be received by mail or fax prior to October 6, 2011. No refunds will be made thereafter.
