J-1 Visa Application Information
There are three different groups of Exchange Visitors who come to UTSW. Each group requires different processing. Please choose the category most appropriate to you from the selections below.
New J-1 Sponsored by UT Southwestern for Research
Items needed from visitor:
- J-1 Application Form - This is an on-line application that the Exchange Visitor must complete and submit.

Curriculum Vitae
- Copy of degree or letter from Dean of university stating that all degree requirements have been met
- If your offer letter indicates you will be appointed as a Postgraduate Trainee or Postdoctoral Researcher/Fellow, provide a credential equivalency evaluation to document that your degree is equivalent to a U.S. PhD degree
- English copy of the health/repatriation insurance from home country declarations page with the coverage amounts converted to US dollars Otherwise, the visitor must purchase insurance in the US. See insurance requirements at Mandatory Health Insurance
- Original funding document, in English, showing dollar amount and dates of funding, if Exchange Visitor is being paid by an organization other than UT Southwestern
Items needed from department:
- J-1 Visa Departmental Application This application is a document that the Exchange Visitor's UT Southwestern department will submit in order to be considered for the J-1 visa.
- 5-Point Letter Supplemental document for visitors who will have hands-on patient contact.
Current J-1 Transferring to UT Southwestern from Another Institution
Items needed from visitor:
- J-1 Application Form This is an on-line application that the Exchange Visitor must complete and submit.
- J-1 Transfer of Program Sponsorship Form Exchange visitor should give this form to their current International Office.
- Additionally, Applicant should submit by fax (214-648-4150) to the International Affairs Office:
- Copy of all previous DS-2019 forms
- Copy of identification page of passport
- Curriculum Vitae (if Exchange Visitor is being paid by an organization other than UT Southwestern)

ECFMG-Sponsored J-1 Clinical Trainee
Update
New process is effective January 1, 2012.Items/actions needed from visitor:
- Wait for an automated email message from ECFMG indicating that International Affairs has completed the institutional portion of the ECFMG J-1 appliation.
- Follow emailed instructions to complete the ECFMG J-1 visa application in OASIS and pay the indicated administrative fee.
- Download from OASIS the checklist of required supporting documents.
- Gather documents and forward to Educational Coordinator of clinical training program, who will then forward them to International Affairs.

Items needed from department:
- ECFMG Sponsored J-1 Visa Departmental Application This application is a document that the Clinical Trainee's department will submit in order to process the necessary ECFMG sponsorship paperwork.
- Copy of Offer Letter signed by Program Director and Trainee.

New J-1 Sponsored by UT Southwestern for Clinical Observation

Items needed from visitor:
- J-1 Application Form This is an online application that the Exchange Visitor must complete and submit.
- Clinical Observation Application to be filled out by Clinical Observer.
- Curriculum Vitae
- Copy of degree or letter from Dean of university stating that all degree requirements have been met.
- English copy of the health/repatriation insurance from home country declarations page with the coverage amounts converted to US dollars. Otherwise, the visitor must purchase insurance in the U.S. See insurance requirements at Mandatory Health Insurance
- Original funding document, in English, showing dollar amount and dates of funding.
Items needed from department:
- J-1 Visa Departmental Application This application is a document that the Exchange Visitor's UTSW department will submit to request issuance of a J-1 visa document for the clinical observer
- Clinical Sponsorship Agreement-GME Completed and signed by mentor (International Affairs will request signature of Director of Graduate Medical Education)
- Clinical Sponsorship Agreement-Hospital Completed and signed by mentor (International Affairs will obtain signature of appropriate hospital authority)
