Tips for Filling out the ARC Security Access Form

1. To request badge access into the animal facilities, please fill out the “ARC Security Access Form.”

2.   Before filling out this form, please verify that you have completed the appropriate training classes.  A chart describing the required training courses is available on the IACUC training website located at
http://www8.utsouthwestern.edu/utsw/cda/dept42789/files/54833.html.

All personnel working with research animals are required to do the following:
• attend the “Humane Care and Use of Animals” class or complete an equivalent online tutorial,
• attend the Pathogen Control Procedures class, and
• enroll in the Worker Protection Program.

Additional training will be required if:
• you must enter the NG Vivarium, Y2 Barrier, or Y3 Biohazard Area;
• you must handle rabbits;
• you have no experience with the species you will be using; or
• you will be required to perform aseptic surgery on rodents and you have no experience doing aseptic surgery in rodents.

To sign up for these classes or to get more information, please call the IACUC office at 5-6423.

All physical plant personnel are required to do the following:
• attend the physical plant training course offered by physical plant, and
• enroll in the Worker Protection Program.

3.    Under the section on the form that reads “IACUC Approval” be sure to enter all of
the IACUC protocol numbers you will be working on.  Physical plant employees should mark this section as “N/A.” 

4.     Please be sure that you and your supervisor have signed the form before submitting
        it to the IACUC office.

5.    After you have filled this form out, please send it to the IACUC office by fax
       (5-6426) or campus mail (mail code 9107).
